The biggest paper of Dr. Marchand’s career and the largest body of patients ever assembled on transabdominal cerclage: 63 studies, 70 study arms and 5,094 women, and the first analysis restricted to within-study comparisons of the laparoscopic, open and robotic routes. Across the six head-to-head studies and 317 pregnancies, no difference was detectable in any complication. Major hemorrhage and uterine perforation were uncommon after laparoscopic placement, with no major hemorrhage in 1,889 laparoscopic procedures; pregnancy loss was not uncommon, and no route was shown to be safer than another. The advantage reported in earlier reviews, which compared two separately pooled literatures, does not reproduce when the routes are compared inside the same studies. A 2021 meta-analysis co-authored by Dr. Marchand comparing salpingectomy with tubal ligation (Mills K, Marchand G, Sainz K, et al. Am J Obstet Gynecol. 2021;224(3):258-265.e4) was cited in the Canadian Medical Association Journal’s practice guidance on opportunistic salpingectomy for ovarian cancer risk reduction (CMAJ. 2024;196(22):E765).
